Clinic Hours: Tuesdays, 2-4:30 p.m. during the school year.
Location:The Kaiser Clinic is located in E100 near the main administration building.
Our school is also looking to retain a traditional school nurse again but is not likely to do so before the clinic becomes operational. |
Important
Notices for Parents & Students
VCUSD
policy for administering medications at school require parent authorization
and MD authorization
for (OTC) over-the-counter and prescribed medications.
In addition to the above, parents are required to provide the medication;
medications must be delivered to the school by the parent or an
adult designee.
-
For
students who self-administer their medication, such as inhalers
used for asthma: a medication authorization form is required
to be on file in the attendance office. Please obtain such an
authorization from the attendance office.
-
For
students who do not self-administer and require assistance:
their medication bottle must be properly labeled with the students
name, doctor, dose, frequency, and route.
Please
submit these forms and medications to the attendance office, for
the attention of the school nurse. Medication authorization forms
expire one year from the date noted on the form by the doctor.
